Insurance Claim Forms

Most insurance companies nowadays have a claims helpline number that is available 24 hours a day, 7 days a week and in the event of a claim you should contact this number. It is normally found in your policy booklet or on your insurance documentation. We have put some of these numbers on our insurance claim numbers page.

Some companies or certain types of insurance, a helpline number is not available in which case a claim form is required. Claim forms must always be completed in ALL sections and returned to us at:
